These investigations were undertaken as part of the Fatality Assessment and Control Evaluation (FACE) program conducted by the National Institute for Occupational Safety and Health (NIOSH). The FACE program was initiated in 1982 and directed from its inception by the NIOSH Division of Safety Research.
