Synopsis "Evaluation of severity and treatment of combined injuries of the pelvis and femur"

The introduction of modern technologies and an orthopedic treatment algorithm, "Damage Control," involving staged treatment of patients with polytrauma, into orthopedics. According to the concept, the first stage consists of minimal lifesaving interventions in the first hours after injury. In the stabilization phase, definitive treatment is carried out. Similar principles under the name "damage control in orthopaedics" ("damage control orthopaedics") are applied when treating patients with concomitant trauma, injuries of long bones and fractures of the pelvis bones. In recent years, the tactics of a traumatologist in the treatment of pelvic bone fractures with concomitant fractures of the femur have undergone a radical change. Conservative treatment is increasingly being replaced by osteosynthesis with external fixation devices. The proposed by the author tactics of treatment and estimation of the condition severity in case of injuries of the pelvis and femur as well as osteosynthesis technique allows to reliably fix the pelvic ring fragments and meets the requirements of functionally stable osteosynthesis. The proposed algorithm for treatment of pelvic bone injuries as well as antishock manipulations can be applied in CDH, township and city hospitals.
