Synopsis "Infektiöse Komplikationen bei offenen Beinfrakturen (in German)"

Offene Beinfrakturen machen 5-10 % aller Frakturen aus. Sie können zu zahlreichen Komplikationen führen, insbesondere zu Infektionen. Die Notfallbehandlung ist medizinisch-chirurgisch. Wir führten eine prospektive Kohortenstudie von Patienten mit offenen Beinfrakturen über einen Zeitraum von sechs Monaten vom 17. Mai bis zum 30. September 2015 durch. Siebenundfünfzig Patienten wurden in die Studie aufgenommen. Die Infektionsrate lag bei 19,3 % (11 Fälle). Als infektiöse Risikofaktoren wurden Gustilo-Frakturen des Typs III (RR = 13,75) und die Exteriorisierung des Knochens (RR = 3,52) identifiziert. Ein sehr hoher C-reaktiver Proteinspiegel von mehr als 100 mg/l in der ersten Woche sagte eine Infektion voraus. Pseudomonas war am häufigsten (33,33%), gefolgt von Staphylococcus aureus (20%). Der Anteil an multiresistenten Bakterien war hoch (40%). Die Empfindlichkeit gegenüber Gentamycin und Cephalosporinen der dritten Generation war gut. Zusammenfassend lässt sich sagen, dass neben der Stabilisierung des Knochens ein chirurgisches Debridement in Verbindung mit einer Antibiotikaprophylaxe unerlässlich ist, um das Risiko auf ein Minimum zu reduzieren.
